Parental Consent:
I hereby consent to the use of products such as: facial cream, lotions, nail polish, quick dry, temporary tattoos, face paint, make-up... on my child(ren). I understand that these products may be used on their face, hands, feet, arms and nails. If I elect to opt out of any portion of the event my child(ren) will be directed to a designated area until completion. I acknowledge that my child(ren) have no medical restrictions (including allergic reactions) that prevent the use of the products utilized during the event. If my child(ren) have any known allergies or medical restrictions, I will specify them on this R.S.V.P. for the host (Birthday childs parents) to exclude my child(ren). I also understand and agree that if my child(ren) should have an adverse reaction to said products Ann R. L. Best Events and its authorized agents will be held harmless and free of liability. Lastly, I understand all authorizing parents (including myself) will be liable and agree to pay all costs and expenses incurred in connection with such medical services (if necessary) rendered to the aforementioned child(ren) pursuant to this authorization.
